If you are using Microsoft Office 2007 or later, click the above link to download a printable version of our required client information form. If at all possible, please type your information into the form and print it out. If not, you may simply print the form and fill it out by hand. Please Note: If you are using Microsoft Office 2010 or later, this file will open in protected mode (you will not be able to type in your responses). Please click the "enable editing" button on the yellow security bar at the top of your window to proceed. Again, you MUST bring this completed form with you the day of your appointment
