If you are a new client, you must complete the following, prior to your appointment:
1. Review each page of the Services tab, including all the links listed under the picture.
2. You MUST fill out the client information form (see the Client Forms link) prior to your appointment. You may either complete the form on your computer and return it back to us via email, or print the form and bring with you completed at the time of your appointment.
